You'll learn how to fill out this form when you start RI Ground School and will be filling it out for most of your later sims and flights. This Word template allows you to make yours look nice even if you have bad handwriting.
Scaled to fit on your kneeboard (you'll have to trim the edges), this Excel template takes your inputs and calculates what it can to minimize your workload (behold the power of automation!). One way to know if an equation is present is a small green triangle in the upper corner of the cell. I recommend removing the borders from the middle space you're not using when you print it out to give you a place to write things down during the flight.
